July 2021 COVID-19 UPDATE
AS OF 4/01/2023, WE ARE NO LONGER REQUIRING OUR DOCTORS AND STAFF TO WEAR MASKS FULL TIME.
As allergy season can still cause sneezing, coughing, etc., we will use masks as needed, to protect each other from any cold symptoms and following any exposure to illnesses from our friends and family members.
WE STILL ASK THAT IF YOU ARE NOT FEELING 100%, PLEASE RESCHEDULE (or at least consider wearing a mask yourself.)
Our office is open during our usual hours (Monday-Thursday 8:15-5:00/ Friday 8:30-1:00) If you are coming in for a visit with one of our doctors, we ask that you bring PHOTO ID, ALL MEDICAL/ VISION INSURANCE CARDS, and ANY GLASSES YOU USE FOR ANYTHING (including : computer glasses, sunglasses, OTC readers, and any pair of other special lenses you may use routinely). If you wear contacts, bring your current boxes as well.

Our paperwork is available through an online patient portal.
COMPLETE THE PATIENT PORTAL BY CLICKING BELOW ON "ACCESS OUR PATIENT FORMS" .
CLICK: "Lets Start"
If this is your first time on the portal, you do NOT need to enter a code, password, or email address to log in.
Once logged in, It will give you the option to create a password. If you make a password at this time, it will allow you to access these pages in the future, without having to enter them all over again.
It is important that you fill out all information boxes thoroughly, as though you are a completely new patient. Even if you have been a patient here for years. Make sure that under "medications" you include ALL medications you take (including: RX and OTC products, supplements, anti-inflammatory meds, infusions, injections, inhalers, eye drops etc.)
Medical History includes all conditions you are receiving treatment/ management for. If you have a diagnosis and you are not treating it, we need that information as well.
If you are unable to access the portal, please contact our front office staff 405-341-7055, so we may make arrangements to assist you before your exam.
